When shipping your Sample back to the United States, Canada Post is recommended.
An Online Customs Form is required by Canada when shipping Internationally. This can be found on Canada Post's Website.
Enter Shipping and Package Details to receive a Digital Barcode.
Present this Barcode either printed or on your mobile device at the post office with your package.
Please use the box containing your collection kit to return your sample
Recipient Details
Enter the below information for Recipient Details for your kit to be returned to the lab, CosmosID Inc.
Package Declaration
Type in Exempt Human Specimen Sample in the other section and select Return to sender.
Package Contents
Type in Exempt Human Specimen in the description. Enter 1 for the other items in this section.
Create Barcode
Print barcode or show barcode on your mobile device at the post office with your package.
